How can a bank resive the Loan Insurance from a nation? I don't see any option for me to request it. Is the payment manual according to what the nation said to do, and people have to trust them? How this works?

I think its auto, and only if the loan is at a certain % left and the maximum insurance value is right, also, the person loaned to must not have paid the loan X days.

i made a trial,

50k loan, so the nation insurance cover the loss. Time limit is over, but still i see no option inside Nation or Bank, and still loan haven't paid by Nation.

Player has to be inactive for 30 days I think.

Aye, like DezNutz said, player has to be inactive for 30 days. after that, nation automatically pays it and i believe the person's debt is transferred to the nation (as in the player now owes X amount to nation Y).
