Noffin isn’t imagining the Advisor, Pitt. He is seeing the apparition that has haunted him for so many years after he killed him. Only to kill the apparition again and again.

Sounds like an extremely ghastly experience...
As you bring that up, petition for CJ to change the present option from "Open your chest" to "Open your gift". The former sounds...painful.
