Basically, he attempted a ritual to turn himself into a lich, using very old magic. But he botched it. His soul, and much of his physical form, besides his clothes and about 69% of his bones were trapped in a pocket void.

Using what little time he had, he used shadow magic, and alchemy, to build himself an artificial soul, thus allowing himself to stay in contact with this reality, as long as he keeps some of the Extract on him at all times.
